- Application form (Must be completed and signed by the applicant.)
- Original copy of the motor insurance policy (The “Guangdong and Macao Cross-Border Motor Insurance” should be subscribed together with motor insurance underwritten by Fidelidade Macau)
- Copy of the applicant's Macao ID (Applicant must be the car owner, i.e. the policyholder. If there is more than one car owner, ID copies of the other owners must also be submitted.)
- Copy of applicant's Mainland Travel Permit for Hong Kong and Macao (Home Return Permit)
- Copy of driver's ID/driving license (If there is more than one driver, copy of the second driver's ID/driving license must also be submitted.)
- Copy of Vehicle Registration & Vehicle Booklet (“Livrete”). (Screenshot from the Macao One Account is accepted but the captured image should be submitted by email.)
